We will gladly submit the bill for professional services received to your health insurance, but you are ultimately responsible for its payment. Insurance co-pays are expected at the time of your appointment. A $50 down payment will be expected from patients that do not have insurance. Be prepared to provide your insurance card at every visit and notify our registration staff of a change in insurance prior to your appointment. We offer a competitive discounted fee schedule for payment at the time of service. No one will be denied access to care due to inability to pay; there is a discounted/sliding fee schedule based on family size and income. Please inquire for more information if you are interested.
